In the field of heath care, this statement could not be more true. The bottom line is that we can\u2019t do it alone. \u00a0In order to succeed in the emerging landscape, a \u201cteam\u201d approach is more important than ever. \u00a0<\/p>\n<p><strong>T<\/strong>:\u00a0 Transition focused.\u00a0 Punctuated care delivery favors a reconstructive mindset.\u00a0 In other words, high risk, high volume, problem prone conditions are typically treated symptomatically, as exacerbations irrupt.\u00a0 Effective transitioning promotes stabilization of chronic disease states across a continuum of care delivery platforms.\u00a0 Networking with hospitals, ECFs, ALFs, HHAs, medical equipment companies, out-patient clinics and non-traditional care delivery options will ensure that the right care is rendered at the right time. Accountable Care Organizations and Medical Home Models have been shown to create significant savings and enhance patient outcomes by incorporating health promotion, stabilization, and wellness concepts. <sub>1<\/sub><\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-7784 aligncenter\" src=\"http:\/\/l_new.local\/wp-content\/uploads\/iStock-912333756-1024x722.jpg\" alt=\"\" width=\"380\" height=\"268\" srcset=\"https:\/\/devblogsite.hooliv.com\/wp-content\/uploads\/iStock-912333756-1024x722.jpg 1024w, https:\/\/devblogsite.hooliv.com\/wp-content\/uploads\/iStock-912333756-300x212.jpg 300w, https:\/\/devblogsite.hooliv.com\/wp-content\/uploads\/iStock-912333756-768x542.jpg 768w, https:\/\/devblogsite.hooliv.com\/wp-content\/uploads\/iStock-912333756-1080x762.jpg 1080w\" sizes=\"(max-width: 380px) 100vw, 380px\" \/><\/p>\n<p><strong>E<\/strong>: Evidence based. \u201cOpinion based medicine\u201d has created inconsistency in outcome. The healthcare of tomorrow is a <em>value<\/em>, not <em>volume<\/em>, based system. Matching patient characteristics with the optimal clinician(s) will minimize redundancy and unnecessary over utilization.\u00a0 To achieve this objective, nurses, physical, occupational, speech therapists, social workers, and clinical managers must be operating transparently, keenly aware of each other\u2019s skills, abilities and commitment to the greater objective of a favorable outcome. We are all accountable for our patient\u2019s outcome. The transition from siloed to coordinated care is upon us. Embrace it.<\/p>\n<p><strong>M<\/strong>: Measured success. \u00a0John William Gardner, author of, <em><u>On Leadership<\/u><\/em>, states, \u201cExcellence is doing ordinary things extraordinarily well.\u201d<sub>4<\/sub> Stay focused on the successful completion of the plan of care. Target your patient centered goals and stay committed to achieving them. Monitor and measure your progress.
